How to submit an accommodations request when documentation is required
When students are ineligible for accommodations through the school verification process, they may still be eligible through the Document Review process. To find out, they must submit documentation with the Eligibility Form to support their need for accommodations. College Board reviewers will review the documentation and determine eligibility for accommodations for College Board tests.
The Student Eligibility Form (.pdf/446K) tells when documentation must be provided. Requires Adobe Reader (latest version recommended). See Documentation Requirements for more information. To submit a request through the Document Review process:
- The student or parent should complete Section I of the Student Eligibility Form, and in most cases, give the form to the school's SSD Coordinator;
- In most cases, the school's SSD Coordinator will complete Sections II and III of the Student Eligibility Form, and forward the Eligibility Form with supporting documentation to the College Board.
- A student/parent may request accommodations without involvement by the school by completing Sections I and III of the Student Eligibility Form, and forwarding the Eligibility Form with supporting documentation to the College Board.
In most cases when documentation is required, scores from nationally normed, individually-administered testing must be submitted. See Documentation Guidelines.
Diagnosis: What the documentation must tell
Documentation must describe the student's functional limitation and the comprehensive testing and techniques that were used to arrive at the diagnosis. An IEP, 504 Plan, and/or doctor's note are not sufficient. For more information about the type of documentation the student or parent must provide, see Documentation Guidelines.
